REQUIRED ATTENDEE REGISTRATION: All individuals attending the conference must pay a registration fee. If total is over $1,500, you will not be able to pay by credit card through the EOA website. You must complete a credit card processing form through the EOA treasurer. You may contact firstname.lastname@example.org in order to do so.
PARTICIPANT REGISTRATION FEES: All meals, lodging, and conference materials are provided during the conference. All rooms are set up in suite style with double occupancy.
CHAPERONE REGISTRATION FEES: include meals, conference materials, and lodging.
When paying conference fees, please select ONE method: credit card, check or purchase order. Send a copy of your registration confirmation and check or purchase order (made out to MAEOPP/EOA) to:

Registration is refundable less a processing charge of 50% of the actual registration fee when received on or before May 17, 2019. No refund requests will be accepted after this date. Any eligible refunds will be reviewed following the conference. Written notice of cancellation must be emailed to:

To provide the conference committee with the most accurate planning numbers, all completed registration forms will be considered as conference attendees and will be responsible for registered payment. All other cancellations must follow the guidelines above prior to May 17, 2019.
Information submitted in this form will:
A. Provide required conference information to prepare the conference name badge.
B. Assist with meal accountability.
C. Establish session and meeting room formats.
D. Identify volunteers who would like to help with the conference.
E. Assist to provide acknowledgment you are registered for the conference.
F. Assist to provide a receipt (via email) when payment has been verified. A receipt will be sent to email address listed on your membership profile OR the registration form.
In order to allow time for the conference committee to process all information, online registration will terminate on April 26, 2019.
